Hospital to Home Discharge Support | Sydney

I provide non-clinical hospital discharge support across Sydney: transport home, medication collection, basic provisions, and settling the patient safely into their home environment. 

Available for same-day and short-notice discharges.

Hospital to Home | $350 incl. GST

Pick-up from hospital, medication collection, arranging groceries if needed, and settling the patient at home.

Extended Support | $680 incl. GST

8 hour block, day or night. For patients who need supervision beyond the immediate discharge window.

24 Hour Discharge Support | $1200 incl. GST

Full day coverage. Can run consecutive or non-consecutive to work alongside existing supports, including respite.

One Week Discharge Support | $5500 incl. GST

Live-in or daily visits for the first week post-surgery or post-discharge, when a patient needs more than a single day of supervision.

Fully Insured | Current Police Check | Working With Children Clearance |First Aid & CPR Certified
